Application form and prospectus can be held on payment of Rs 1000/- at the counter from; Sri Guru Ram Das Institute of Medical Sciences and Research, Mehta Road, Sri Amritsar-143501; Sri Guru Ram Das Institute of Dental Sciences and Research, GT Road, Sri Amritsar-143001; Sri Guru Ram Das Charitable Hospital, near Gurudwara Shaheedan, Sri Amritsar; SGPC sub-office, Kothi no 30, sector-5, Chandigarh; Guru Nanak Engineering college, Ludhiana or by sending letter through registered post/speed post only, and not by courier, to any of the above along with a demand draft for Rs 1200/- in favour of Registrar examinations, AUSMMDI payable at Amritsar
